Using the Correct DVA Medical Grade Footwear (MGF) Prescription Form
(Information for Podiatrists and Other Assessing Health Providers)
We sometimes receive old versions of Department of Veterans' Affairs Medical Grade Footwear Prescription forms (Form D0688) from as far back as 2009 or even 2005. These forms must no longer be used and will no longer be accepted at Just Comfort Shoes. The current forms were introduced in late 2015 and can be found here.
As a guide, if the form doesn't include "D0688 1215" near the bottom right of most pages, it is an outdated form that should no longer be used. The newer forms can be filled in on screen in the PDF file before being printed out, or can be printed out and filled in by hand. They do not need to be submitted to us with carbon paper duplicates by mail. They can be scanned or photographed and then sent as attachments using the form on our Contact page.
Digital files of scanned or photographed foot tracings may arrive at our shop at a size slightly different from any original foot tracing on paper, but we can print it out to scale as long as measurements are drawn in on the tracing. To do this, please rule one line each across the widest part and the longest part of the foot's outline and write in the measured length of the lines.
